Are you a Carer?
“Do you look after a relative, friend or neighbour who is ill, frail, disabled, has a mental health concern or problematic substance use and who could not manage without your help? If so, then you are a Carer and there are a number of ways you can get help and support.”
Meddygfa Teilo recognises that Carers contribute a significant saving to the NHS by looking after relatives, neighbours or friends who might otherwise have to go into long term care. Young Carers, under the age of 18 may be particularly vulnerable due to their inexperience and lack of life-skills. This practice is committed to providing appropriate support and consideration to its patients who are Carers whatever their age.
How Can You Register As A Carer?
Registration form is available below or in the surgery, alternatively you can speak to any staff member. Forms are also available from other voluntary organisations. Once the form is completed, please return to the surgery in the first instance, who will then, if certain boxes have been ticked, forward this on to the Carers Information Service, you do not need to send it to them directly.

What Can We Do To Help?
The Practice holds a list of those patients who are Carers and who have agreed to be on the register. The register enables the practice to provide Carers with information on local and national support and help. We can also arrange a referral to Social Services for a Carers Needs Assessment and other voluntary service agencies if required. At Meddygfa Teilo we aim to raise awareness of the needs of Carers and to support them with practical help and information.
What Is A Carer's Needs Assessment?
A Carer’s Needs Assessment is your opportunity to tell Social Services about the things that could make caring easier for you. It is an assessment of your needs, not those of the person you are caring for. Every Carer is entitled to an assessment. When you register with us as a Carer, the practice will offer you the chance of a referral.

Bowel Screening Wales
Information for Carers
Bowel cancer is the third most common cancer in Wales. Bowel screening aims to find cancer at an early stage when treatment is likely to be more effective. Early detection is key. At least 9 out of 10 people will survive bowel cancer if it is found and treated early.
Bowel screening is currently offered to people who are aged between 55 and 74 and live in Wales. We get address information directly from GPs. It is important that the person’s correct home address is registered with the GP. We do not hold any medical information.
Benefits of screening
- Finding bowel cancer early gives a person the best chance of survival.
- At least 9 out of 10 people survive bowel cancer if it is found early.
- Bowel screening can help find bowel cancer early, even if a person is well and does not have any symptoms.
- People can complete the bowel test kit at their place of residence.
- The bowel screening test kit is quick and easy to do, with just one small sample of poo needed.
As a carer you may have your own personal views about taking part in screening. Your view may differ from that of the person you care for and you may feel that the person you care for is not making the ‘right’ decision.
You must remember that your role is to provide the person with the correct information (in an appropriate format) and support, so that they can make an informed decision to take part in screening or not. The form below may help you to provide this informed information.
New Developments for Carers
“From 1st April 2023 the Carmarthenshire Carers Information and Outreach Service will transfer over to a new provider Adferiad Recovery and will be called Carmarthenshire Carers Information Services”
Carers Trust Crossroads West Wales (CTCWW) will continue to support carers of all ages by offering a range of other services to them and the individuals they care for. These include:
- Crossroads Care / Resilience Project – Replacement Care and Regulated Care – providing carers with a break from their caring role.
- Young Carers Support Service – one-to-one support, workshops, training, group peer support, clubs, and activities for carers age 5 – 25 years.
- CATCHUP Welfare Benefits Advice Service – benefit advice, checks, form filling and Appeal Tribunal Representation.
- Hospital Discharge Support Service – help with communication with medical staff when the person you care for is in hospital and support to help carers understand their rights and to be involved in discharge planning.
- Money Matters Service – help to maximise income, improve budgeting skills, access to workshops, grants, discounts, and concessions.
- Independent Professional Advocacy as part of the 3CIPA Service – advising carers of their rights, support during assessments and/or care planning reviews, complaints, and safeguarding concerns.
- Children & Young People’s Clubs – supported and fun play opportunities for children & young people with complex needs, learning disabilities and autism.
Cathen Dementia Social Group
A warm welcome awaits you at Cathen Dementia Social Group we are a support service for people living with dementia and their carers.
The group meets weekly and shares stories, has a cuppa, plays games, goes on trips and we laugh a lot, give information and occasional speakers on specific subjects.
The group meets every Thursday in Llangathen Hall, Llangathen SA32 8QH at 2pm

- As a carer looking after your own health and wellbeing is important and becoming smoke-free can give you the extra energy you need.

- No need to travel to appointments and flexible times for phone calls to fit in with your caring responsibilities.
- You will get a call to arrange your first telephone appointment with your advisor to discuss options for NRT products or medication, plus advice to help with any cravings during your quit.
- There will also be ongoing telephone support from a trained advisor to help you to stay focussed on your goal and to talk through any challenges along the way.

Carers UK: Looking After Someone
The Carers UK Looking after someone guide is divided into the following sections: getting help and support, your finances and your work.
The guide includes:
- This edition includes A Carer’s Guide: an illustrated introduction to the challenges of caring, from making difficult decisions to looking after your health and wellbeing.
- Benefits: an overview of which benefits you or the person you care for may be entitled to and information about how to get a benefits check.
- Other financial help: including help with council tax, fuel costs, pensions and health costs.
- Practical help: including community care assessment, carer’s assessment and direct payments.
- Technology: information about health and care technology that could make life easier for you and the person you care for.
- Your workplace: your rights at work, from flexible working and parental leave to protection from discrimination.
- Other help: how to find other help nationally and in your local community.
The Education Programme for Patients based in Hywel Dda Health Board offer free self-management courses for carers aged 18 and over. All courses are available in community venues throughout Carmarthenshire, Ceredigion and Pembrokeshire as well as virtually using Microsoft Teams.
The courses focus on reminding the carer to look after themselves as well as the person they care for, as well as offering information to help support them in their caring role. More information can be found here.
We also offer courses for anyone aged 18 and over living with a long term health condition. A list of our courses can be found here in Welsh or here in English.

Young Carers Service
The national Young Carer I.D. Card, funded by Welsh Government, is now available in Carmarthenshire, Ceredigion and Pembrokeshire. This enables a coordinated regional approach in supporting young carers, making it easier for them to access the help they need, when it is needed.

Carers Grants for Unpaid Carers
Newyddion gwych! Mae Llywodraeth Cymru wedi darparu Grant Gofalwyr ar gyfer Gofalwyr di-dâl o bob oed yng Sir Gaerfyrddin, Ceredigion, a Sir Benfro. Mae grantiau ar gael ar gyfer talebau bwyd gwerth £100, nwyddau gwynion / eitemau hanfodol (hyd at £300), cymorth iechyd a lles ac atgyweiriadau hanfodol i’r cartref.
Great news! The Welsh Government have made available Carers Grant for unpaid Carers of all ages in Carmarthenshire, Ceredigion, and Pembrokeshire. There are grants available for £100 food vouchers, white goods / essential items (up to the value of £300), health and wellbeing support and essential household repairs.
